Elders Outreach and Care Worker


(Full Time Regular)


 


The Elders Outreach and Care Worker will play a crucial role in fostering and building strong relationships with Elders within the Tsawwassen First Nation community. This position involves regular check-ins and providing support in ensuring the overall wellbeing of our Elders. The worker will be responsible for offering care assistance, monitoring mental health, facilitating service access, providing transportation, and advocating for Elders who need additional support. The role also includes documentation and record-keeping of any issues, incidents, or needs that arise.


 


Specific Responsibilities



  • Conduct daily, weekly, and monthly check-ins with Elders, based on individual needs

  • Provide transportation for Elders to appointments, activities, and other required locations

  • Perform basic care checks and mental health check-ins

  • Assist Elders in accessing necessary services and community resources

  • Foster a strong connection between Elders and the community

  • Ensure the wellbeing of Elders by advocating for their needs and acting as a liaison

  • Help Elders in voicing their concerns and accessing support when necessary

  • Maintain accurate records and documentation of any incidents, concerns, or care needs

  • Support Elders with dementia, mental health, palliative care, and other health-related issues


Qualifications



  • Proven experience working with First Nations and Indigenous communities

  • Certification as a Care Aide or equivalent healthcare training

  • Experience working with Elders and Seniors, particularly in a caregiving capacity

  • Knowledge and experience in supporting individuals with dementia, mental health conditions, palliative care, and other Elder-specific health concerns

  • Strong communication and interpersonal skills

  • Compassionate and patient attitude with the ability to engage with Elders on a personal level

  • Organized and detail-oriented with the ability to maintain accurate documentation


Knowledge, Skills & Experience



  • Strong understanding and appreciation of TFN culture, traditions, and the unique needs of Elders

  • Excellent organizational, administrative, and project management skills

  • Compassionate and empathetic approach to working with Elders

  • Effective communication and interpersonal skills

  • Ability to work independently and as part of a team

  • Proficiency in Microsoft Office and other relevant software


 Other Requirements:



  • Must have a valid driver's license, Class 4 an asset

  • Ability to work flexible hours, including evenings and weekends, as needed


Working Conditions 



  • Full-time position

  • Regular travel within the community may be required

  • On-call availability may be necessary to respond to urgent Elder needs


 


This role is vital in providing essential care, fostering community connections, and supporting Elders to live comfortably and safely within the Tsawwassen First Nation community.


 


Wages: Pay scale for this position is $32.57 to $34.29 per hour with excellent benefits including enrolment in the BC Municipal Pension Plan. Placement within the pay scale is based on a review of skills, experience & internal equity.
