The Primary Nurse Clinician & Supervisor, Integrated Programs is a key clinical leader responsible for guiding a team of Registered Nurses (RNs), Registered Practical Nurses (RPNs), and Unregulated Care Providers (UCPs) to deliver advanced, best-practice care in home and community settings. This role combines clinical excellence, leadership, and mentorship to support sustainable care transitions, hospital avoidance, and optimized patient outcomes.
The Primary Nurse Clinician & Supervisor, Integrated Programs will leverage an advanced nursing skill set to deliver time-sensitive, comprehensive patient visits—often within 24 hours—to stabilize complex patients, conduct thorough assessments, and develop or adjust individualized care plans. They will identify additional service needs and coordinate with the broader care team to ensure seamless and high-quality care delivery.
In addition to direct care, the Primary Nurse Clinician & Supervisor, Integrated Programs will play a critical role in training and upskilling team members to deliver more acute and advanced care in the community. They will drive clinical excellence through comprehensive documentation, care plan development, and the effective sharing of critical information across the team, ensuring care continuity, safety, and adherence to best practices.
This role emphasizes patient-centered care, team collaboration, and continuous quality improvement to meet the evolving needs of complex patients and transform the delivery of care in home and community settings.

Key Responsibilities
- Possess and apply an advanced nursing skill set to manage complex patients and situations.
- Train and mentor the care team to deliver more acute and advanced care in the community, enhancing their clinical competencies.
- Provide direct, comprehensive patient care during time-sensitive visits, often within 24 hours, to stabilize patients, complete assessments, and implement or adjust care plans as needed.
- Conduct thorough, evidence-based assessments of complex patients to inform care planning and decision-making.
- Develop, implement, and adjust individualized care plans in collaboration with patients, families, and interdisciplinary teams.
- Identify additional service needs and ensure appropriate resources are mobilized to optimize patient outcomes.
- Lead comprehensive documentation processes, including care plan development and the completion of required clinical modules, to ensure clarity, accuracy, and compliance with regulatory standards.
- Share critical patient information across the care team to support continuity and safety.
- Provide ongoing support, education, and supervision to the care team to foster clinical excellence and professional growth.
- Promote a culture of learning, collaboration, and accountability within the team.
- Champion innovative, patient-centric care models that prioritize safe transitions, hospital avoidance, and sustainable care delivery.
- Ensure services meet quality standards and align with organizational values of continuous improvement and best practices.
- Possess a current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse
- 3-5 years of supervisory experience or equivalent clinical leadership experience in home and community healthcare.
- Be able to establish and build rapport with clients and Care Staff effectively
- Advanced Clinical Expertise - Demonstrated ability to deliver acute and advanced care in the home or community setting
- Strong Leadership Skills - Proven track record in team development, supervision, and fostering collaboration.
- Exceptional Organizational Skills - Capacity to manage time-sensitive visits, prioritize patient needs, and maintain comprehensive records.
- Commitment to Quality Care - Passion for delivering high-quality, safe, and patient-centered services.
- Strong proficiency with Microsoft Office Suite and experience with scheduling/documentation systems.
- Possess a valid Ontario driver’s license with access to a reliable vehicle
- Have the ability to provide the best possible solutions to problems that align with policies and uphold best practices
- Be self-motivated with an ability to work both independently and as an effective member of a team
