You will be responsible for providing direct quality personal care to patients in accordance with their respective care plan. Primary Responsibilities
- Practices within an Interprofessional Collaborative Practice (IPC) philosophy and model of care.
- Demonstrates BCHS policies and procedures to guide their clinical practice.
- Collaborates with the interprofessional care team in clinical discussions related to observed patient care, needs, and outcomes.
- Collaborates with the interprofessional care team to provide relevant patient information when and where required.
- Collaborates with the interprofessional care team and Registered Nursing staff to ensure the patients established activities of daily living (ADL).
- Assists in the care process by observing and reporting changes in patients physical and emotional condition to the regulated healthcare provider
- Assists patients with ADLs such as personal care, toileting, feeding, and recreational activities as noted in the care plan.
- Assists patients with transfers and ambulation through the use of walkers, canes, wheelchairs, and other supportive devices as noted in the care plan.
- Documents patients care needs as per BCHS documentation policy and procedure.
- Personal Support Worker (PSW) Certificate from a Community College.
- Current BCLS Certification
- Recent hospital experience preferred.
- Demonstrated ability to function as an effective team member.
- Excellent interpersonal, written and oral communication skills.
- Demonstrates excellent critical thinking, problem solving, conflict management, and leadership and skills
- Demonstrated time management and organizational skills.
- Demonstrated ability in working with a variety of patient populations within a hospital setting.
- Safety (patient, worker & workplace) is a BCHS Corporate Priority. The successful applicant will demonstrate good stewardship in the identification, reporting and mitigation of unsafe acts or conditions.
- Reliable individual with excellent attendance record and able to work all shifts
