Biweekly Hours: 75 Travel: Yes Location: Remote Position: The candidate must reside in Ontario or Nova Scotia. The position is remote but requires on-site attendance periodically at sites across Ontario or Nova Scotia. Job Summary: The Professional Practice and Quality Consultant - Skin and Wound Care Practice Lead provides leadership through the application of advanced nursing knowledge, skills and judgment specific to integumentary, wound, ostomy and continence care. The Consultant develops quality improvement initiatives, including clinical and professional practice policies, guidelines, and tools, required to enable safe and effective client- and family-centered care. The Consultant utilizes a variety of resources to enhance service delivery programs and inform clinical and professional practices at VON through the review and implementation of clinical best practice guidelines, regulatory standards, and research-based evidence. Key Responsibilities:

  • Lead the development and implementation of an innovative Skin and Wound Care Program.
  • Develop a monitoring system to track capacity-building initiatives and progress towards identified targets.
  • Identify and participate in quality improvement opportunities through internal data collection sources to develop a data-driven culture in skin and wound care.
  • Performs quality audits to identify inefficiencies, non-compliance issues, and areas for improvement by providing regular reports to Chief Nursing Officer and other leaders when required.
  • Analyze data for emerging evidence and trends on wound care and prepares position papers, briefs and reports as required.
  • Ensure adequate and qualified support of care provider objectives and industry best-practices.
  • Continuously review and update clinical standards ensuring they align with best practices in care plan development and assignment, client education, electronic health record, medication monographs and libraries, and care directives.
  • In collaboration with other stakeholders, support clinical product selection, train the trainer education and knowledge sharing of key clinical elements of Skin and Wound Care.
  • Address end user challenges and identify effective remedial actions to enhance system performance and user experience.
  • Collaborate with trainers and end user stakeholders on practice issues.
  • Engage regulated and unregulated care providers to optimize client care and service delivery while driving quality improvement initiatives.
  • Develop and maintain expert clinical knowledge and skills in Skin and Wound Care.
  • Champions as the skin and wound care expert on the PQR team.

Education, Designations and Experience:

  • Baccalaureate degree in Nursing required.
  • Current registration with the provincial nursing regulatory body required
  • Minimum of 5 years of recent, relevant clinical nursing experience in a home care setting.
  • Minimum 3 years recent experience in Skin and Wound practice and education is preferred.
  • Canadian Nursing Association (CNA) certification in wound, ostomy and continence care (WOCC(C)) is preferred.
  • Post Graduate certification in Skin and Wound Care may include one of the following programs and consideration may be given to applicants enrolled and in progress in any of the following programs is preferred.
  • Master of Clinical Science in Wound Healing
  • Master of Science in Community Health and Wound Healing
  • Master of Science in Wound Healing and Tissue Repair
  • Nurses Specialized in Wound, Ostomy and Continence Canada (NSWOCC)
  • International Interdisciplinary Wound Care Course (IIWWCC)

Skill Requirements:

  • Demonstrate proficiency with the use of clinical and workplace technology including but not limited to Electronic Health Records, etc
  • Proficiency in Microsoft Office applications, including Word, Excel, and PowerPoint.
  • Demonstrate commitment to ongoing professional development
  • Demonstrated commitment to working in an environment with high confidentiality and discretion.
  • Strong management skills and the ability to lead, motivate and support both a core and an extended team of staff and volunteers.
  • Superior interpersonal skills, including the ability to relate effectively with people at all levels (internal and external).
  • Excellent verbal and written communication skills.
  • Able to work both independently and within a team.
  • Strong organizational skills with an ability to prioritize and multi-task.
  • Must demonstrate strong attention to detail.

Other:

  • Driver's License and travel within the province is required
  • A current and original copy of a satisfactory Criminal Records Check.
  • A Vulnerable Sector Search and/or Child Abuse Registry Check may be required
  • The use of Personal Protective Equipment (PPE) may be required.

Working conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
