Position Summary
The Program & Project Specialist supports the strategic design and day-to-day implementation of Integrated Care Pathways across East Toronto. This role will work closely with clinical, operational, and community partners to co-design and execute pathway priorities—ensuring work is aligned, actionable, and making measurable impact.
This role combines program-level thinking (alignment, engagement, change management) with project execution (tracking, coordination, communication) to support ETHP's integrated care transformation. The ideal candidate thrives in dynamic, multi-stakeholder environments and can balance the big picture with tactical detail.

Key Responsibilities
Program Coordination & Implementation
- Contribute to the planning and design of integrated care pathways in alignment with ETHP and Ontario Health priorities.
- Ensure coordination and integration with other ETHP initiatives (e.g., primary care attachment, neighbourhood care, home care modernization).
- Support ongoing partner and stakeholder engagement across sectors, including coordination of meetings and co-design sessions.
- Apply a health equity lens to planning and execution.
- Monitor progress toward milestones and ensure timely delivery of program deliverables
- Develop and maintain detailed project work plans, dashboards, and tracking tools for multiple pathway initiatives.
- Monitor timelines, risks, and dependencies across initiatives; flag issues and propose mitigation strategies.
- Prepare high-quality project materials including presentations, reports, and implementation updates.
- Organize or support meetings (e.g., steering, working groups, clinical tables), including agendas, logistics, materials, minutes, and follow-up.
- Use visual storytelling and data design to communicate insights effectively
- Apply and promote project management tools, best practices, and templates
- Conduct data collection and perform descriptive statistical analysis to assess performance
- Design and implement data tools (e.g., surveys, interviews, focus groups)
- Analyze and synthesize qualitative and quantitative data to generate actionable insights
- Work with EMRs, hospital databases, and data platforms to ensure accurate tracking
- Build frameworks for monitoring, evaluation, and continuous improvement
- Support collaboration with research and evaluation to integrate data and feedback loops into pathway work.
- Translate data and insights into user-friendly tools (e.g., dashboards, infographics, reports) to support decision-making and knowledge sharing.
- Contribute to quality improvement processes and continuous learning within the pathways.
- Work with ETHP communications to develop materials for diverse stakeholders including patients/caregivers and providers.
- Support change management efforts with clinicians, administrators, and partners
- Navigate complexity and ambiguity with strong facilitation and communication skills
- Build and maintain strong, trust-based relationships with ETHP partners and stakeholders
- Ensure initiatives align with ETHP’s broader work, including:
- Home and community care modernization
- Neighbourhood care strategies
- Primary care attachment and access
- Participate in ETHP Virtual PMO and relevant working groups to ensure integration across initiatives
- Other responsibilities may be added as ETHP continues to evolve as an accelerated OHT
- Required: Bachelor’s degree in public health, health sciences, health administration, or a related field
- Preferred: Graduate degree or additional training in project management, quality improvement, or health systems.
- Assets: Project Management certification (e.g., PMI) or equivalent experience
- Project Management: 3–5 years’ experience leading or coordinating complex projects in healthcare
- Health System Knowledge: Strong understanding of Ontario’s health system, local health issues, and OHT model; experience with EMRs and health data systems preferred
- Data & Evaluation: Experience with performance measurement, data analysis, and continuous improvement approaches
- Engagement: Exceptional interpersonal and communication skills, with experience engaging diverse interested parties
- Change Leadership: Ability to support and lead change in complex environments
- Technical Proficiency: Skilled in Microsoft Office Suite, Visio, and data visualization tools (e.g., Power BI, Tableau)
- Communications: Strong verbal and written communication skills to deliver simple and concise messaging to a range of partners
