Our team
Community Outreach Services (COS) provides an Intensive Case Management model of care to individuals experiencing homelessness, housing insecurity, live with substance use challenges with or without mental health problems, often have histories of trauma and other complex care needs and experience barriers in receiving care that interfere with their ability to live safely and independently in the community.
COS is a collaborative multidisciplinary team consisting of social workers, outreach supports workers and a community Resource coordinator. The team works alongside their clients to provide a range of services such as street outreach, advocacy and brokerage of services including access to health, housing, income and other identified community resources. The primary goal is to support clients to exit homelessness in a sustained fashion, maintain housing stability and improve overall quality of life and independence .

Essential qualifications
Please submit your resume clearly demonstrating how you meet the qualifications below. Selections for further consideration will be based solely on the information you provide in your resume.
- Bachelor of Social Work degree from a recognized post-secondary institution
- Significant experience providing Intensive case management for vulnerable populations
- Experience supporting clients with complex issues including substance use, mental health disorders, medical, financial and legal challenges and cognitive impairments
- Significant experience working with a multidisciplinary collaborative team environment.
- Experience and knowledge of the principles of Trauma-Informed care, Harm Reduction, Cultural Safety, Housing First, and Assertive Engagement.
- Experience in crisis intervention and crisis prevention
- Experience working with legislative programs would be considered an asset

- Complex case management services for vulnerable clients experiencing homelessness, housing insecurity, substance use and mental health disorders, and cognitive impairment.
- Bio-psychosocial assessments to determine program eligibility
- Developing, implementing, and evaluating case plans in collaboration with clients, and other formal and informal supports.
- Collaborative engagement and practice with community partners in the homeless serving sector to ensure a coordinated system of care for high-acuity clients
- Outreach, case management, counselling, and linkages supports and services.
Conditions of employment
You must meet the following conditions before being hired, or within the time frame noted.
- Vulnerable Sector RCMP security clearance
- TB screening
- WHMIS 2015
- Valid Class 5 Driver’s Licence.
This job requires you to perform, and/or be exposed to, the following:
- To provide complex case management, including outreach supports to vulnerable individuals experiencing homelessness and housing instability and intersecting complex needs.
- Works primarily in an office setting but there is regular some community outreach involved.
- Exposure to clients with high levels of complex needs.
